Membership Plan Pricing:
$399 per year – Individual Plan
$350 per year – Each additional family member (age 12 and older)
$275 per year – Each additional child (under age 12)
This plan is valid for one fiscal year upon payment and cannot be combined with dental insurance or other discount programs. It is available exclusively at Lindenhurst Dental Health Group in Lindenhurst, IL. The “20% off all other treatments” discount is based on our Usual, Customary, and Reasonable (UCR) fee schedule. A “basic cleaning” is considered a prophylaxis (D1110), valued at $124.

For patients requiring a periodontal (deep) cleaning or periodontal maintenance, the value of a basic cleaning (D1110) will be credited toward the treatment twice per year, and the remaining balance will receive a 20% discount.
